Appointments of patients with a referral from another hospital

If you have a referral from another hospital, you can make an appointment of an examination over the phone.
To do so, call the Medical Cooperation & Patient Support Center.

Follow-up examination procedure

* If it is not your first time to visit the hospital but it is your first time for a specific department, you must still go through initial examination reception.

Using the return-visit reception machine

For follow-up examinations, you can complete the procedures with the return-visit reception machine.
Insert your hospital ID card into the return-visit reception machine, and follow the on-screen guidance.
* If you do not understand how to operate the machine, ask the general information desk.

Patients undergoing their initial examination

Reception procedure

At the table, fill out the required information on the examination application and pre-screening medical questionnaire (name, date of birth, current address, phone number, etc.). (You can download the examination application and pre-screening medical questionnaire from here.) (Note) On the day of the examination, the department examining you could change. We appreciate your understanding.
Take your examination application, health insurance card, pre-screening medical questionnaire, and if available, letter of referral, to (1) Examination Reception.
Once the procedures are completed, we will give you your hospital ID card, outpatient slip, and reception slip. Take these to the department performing the examination. Your hospital ID card will be required for the next examination so make sure to not lose it.

If you do not know which department is performing your examination, feel free to ask at the general information desk.
If it is not your first time to visit our hospital but it is your first time for a specific department, you must still go through initial examination reception. Fill out the examination application and go to (1) Examination Reception.
When the initial examination costs are calculated, if you do not have a letter of referral (patient referral document) from another medical institution, you will have to pay a 3,300 yen (including tax) “Sentei Ryoyo hi” that is additional fee for a patient without a referral at the time of payment.
Also, the oral surgery department is handled differently under the Health Insurance Act from other medical departments. There will be a separate additional fee.
If you are transported by an ambulance or receiving publicly-funded health care, you will waive the additional fee even without a letter of referral.

Ask the staff if you have questions regarding the above. We kindly request your understanding.
The additional fee has been imposed according to the system established by the MHLW for initial examinations to be provided by clinics and advanced, specialized medical care to be provided by hospitals. Accordingly, if you do not have a letter of referral from another clinic or hospital for your initial examination and you are examined by a hospital with at least 200 inpatient beds, the fee will be charged in addition to the initial examination fee.

The follow-up examination reception hours are 7:45 a.m. to 11:30 a.m.

Reception procedure

Insert your hospital ID card into the return-visit reception machine, and follow the on-screen guidance to complete reception. Your outpatient slip and reception number slip will be printed. Insert the outpatient slip and examination ticket into the green file, and submit it to the examining department. (The number on the reception number slip will be called by the corresponding department.)

Notes

For follow-up examinations, you can complete the procedures for the examination with the return-visit reception machine.
Afternoon examinations are depending on the days and examining departments. Contact the corresponding department.

Treatment fees and payments

Treatment fees not covered by health insurance

Treatment fees not covered by health insurance are handled in the following way. Vaccination shots, cosmetic surgery, etc., have individual fees. Contact us for details.

Payments with a credit card

Credit card

You can pay for medical fees with a credit card. The following cards can be used.

You can use a credit card in the automatic settlement machine and at the payment counter. If you use the automatic settlement machine, you must pay in one-time installment.
